40 year old lady married for 12years, presented as a case of primary infertility
According to lady, she had undergone one IVF cycle, at Chennai which failed owing to difficult transfer.
In view of above history and during fresh workup of the patient at our centre, mock transfer was attempted on D2 of menses but could not be done due to severe resistance at the level of internal OS.
Hence, she was taken up for gynaecoscopy under short acting GA. The scope could not be negotiated beyond internal OS. Following which, cervical shaving was done. Then in subsequent cycle, IVF was done and with blessings of God, it was successful. Her first trimester was uneventful & she underwent prophylactic cervical encirclage at 14 weeks of pregnancy.
At 30 weeks of gestation, she presented with PPROM and pain abdomen. Initially managed conservatively with antibiotics & steroid coverage. On day 3 of admission, her CRP was raised & she had low grade fever. On USG OBS, there was no liquor, hence emergency LSCS was done and an alive pre-term male baby of 1.16 kg was delivered.
The baby was kept in our centre’s NICU on nasal CPAP for moderate RDS for 3 days, to which baby responded well. On day 4 baby developed NEC grade 1, managed conservatively. Later feeding hiked gradually and after 20 days was shifted to mother side for rooming in and kept under observation for next 15 days.
Then with a weight of 1.4kg and after ROP screening, finally the baby was discharged and on follow-up after 1 month, the baby weighed 1.9kg and was doing well.
Now Laparoscopic Radical Hysterectomy ( Cancer Surgery ) is possible at our centre at Kota
